PathwayNotes Assessments: AUDIT, PCL-5, PSS-10, PHQ-9

The Assessments Tab

Clinical assessments are built directly into PathwayNotes. Send them from here, track whether they've been completed, and review scores — without leaving the platform.

Sending an Assessment

Tap Send Assessment to open the selector, or use the clipboard icon in the Messages tab. The client receives it as a card in their iPhone message thread with a Fill Out button. Once they complete it, the result appears here.

Available Assessments

AUDIT: Alcohol Use Disorders Identification Test — 10 items, screens for problematic alcohol use.

PCL-5: PTSD Checklist for DSM-5 — 20 items, measures PTSD symptom severity.

PSS-10: Perceived Stress Scale — 10 items, measures subjective experience of stress.

PHQ-9: Patient Health Questionnaire — 9 items, screens for depression severity.

Scores are displayed numerically. Trend visualization across time is on the product roadmap.

For how clients experience receiving and completing an assessment, see 'Messages — Staying in Touch With Your Therapist' in For Clients.
